Responsibilities
- Develop and implement comprehensive, evidence-based care plans for high-risk individuals and families.
- Conduct thorough psychosocial assessments to identify barriers to self-sufficiency.
- Facilitate cross-agency collaboration to ensure seamless service delivery and resource access.
- Maintain precise, high-quality clinical documentation in compliance with state and federal regulations.
- Provide crisis intervention and conflict resolution support in high-stress scenarios.
- Mentor junior case managers and interns to foster organizational excellence.
- Advocate for client needs within legal, medical, and housing systems.
Qualifications
- Master’s degree in Social Work (MSW), Counseling, or a closely related human services field.
- Valid LCSW or LSW licensure in the state of Illinois.
- Minimum of 3+ years of professional experience in social services or case management.
- Proven ability to manage a diverse caseload with complex socioeconomic challenges.
- Exceptional verbal and written communication skills for documentation and advocacy.
- Deep understanding of trauma-informed care principles and community-based resources.
- Proficiency in electronic health record (EHR) systems and case management software.
