Responsibilities
- Develop and implement comprehensive, trauma-informed care plans for families and individuals.
- Conduct thorough psychosocial assessments and monitor ongoing client progress through home visits and office meetings.
- Maintain detailed, compliant electronic medical records and case documentation in accordance with state regulations.
- Collaborate with local healthcare providers, schools, and government agencies to coordinate wrap-around services.
- Provide crisis intervention and conflict resolution support for clients experiencing acute housing or financial instability.
- Mentor junior case workers and provide clinical insight during weekly multidisciplinary team meetings.
- Advocate for client needs within legal and social service systems to ensure equitable resource allocation.
Qualifications
- Master’s Degree in Social Work (MSW), Psychology, or a closely related human services field.
- Valid LCSW or LSW licensure in the State of Illinois required.
- Minimum of 3 years of professional experience in direct client case management.
- Proven ability to work effectively with diverse populations and high-risk demographics.
- Exceptional verbal and written communication skills for reporting and advocacy.
- Strong proficiency in digital case management software and Microsoft Office Suite.
- Ability to demonstrate cultural competency and deep empathy in high-pressure environments.
