You will play a pivotal role in designing holistic care plans, managing complex caseloads, and fostering community resilience. Responsibilities
- Conduct comprehensive bio-psycho-social assessments to identify client needs and goals.
- Develop, implement, and monitor individualized service plans in collaboration with community partners.
- Provide crisis intervention and support services to individuals facing housing, health, or financial instability.
- Maintain precise, confidential electronic case records in compliance with HIPAA and state regulations.
- Coordinate with local non-profits, healthcare providers, and legal entities to streamline service delivery.
- Advocate for client needs during interdisciplinary meetings and community resource committee sessions.
- Provide mentorship and clinical guidance to junior case managers and interns.
Qualifications
- Master’s degree in Social Work (MSW) or a related Human Services field required.
- Current LCSW or LSW licensure in the state of Illinois is highly preferred.
- Minimum 3 years of experience in social casework or community-based advocacy.
- Deep understanding of the Chicago social services landscape and local resources.
- Exceptional verbal and written communication skills with the ability to build rapport with diverse populations.
- Proven ability to manage high-volume caseloads with accuracy and empathy.
- Proficiency in case management software and Microsoft Office Suite.
