Responsibilities
- Develop and implement comprehensive, long-term case plans for clients facing housing and economic instability.
- Conduct thorough biopsychosocial assessments to identify barriers to client self-sufficiency.
- Maintain precise, HIPAA-compliant documentation and clinical records for all active cases.
- Facilitate interdisciplinary team meetings to coordinate holistic support services.
- Advocate for client rights and access to public benefits, mental health resources, and vocational training.
- Monitor progress through regular home visits and community-based follow-ups.
- Provide crisis intervention and de-escalation support during high-stress situations.
Qualifications
- Master’s degree in Social Work (MSW) or a related human services field.
- Current LICSW or LCSW licensure in the state of Washington.
- Minimum of 3 years of direct case management experience with diverse urban populations.
- Proven expertise in trauma-informed care and crisis intervention methodologies.
- Advanced proficiency in electronic health record (EHR) systems and data reporting.
- Valid driver’s license and access to reliable transportation for local travel.
- Strong interpersonal skills with the ability to maintain professional boundaries in challenging environments.
