Responsibilities
- Develop and implement comprehensive, individualized service plans for clients navigating complex social challenges.
- Conduct thorough assessments to identify barriers to stability, including housing, mental health, and financial security.
- Provide crisis intervention and ongoing emotional support to families in high-stress situations.
- Coordinate with local non-profits, governmental agencies, and medical providers to ensure a multidisciplinary approach to care.
- Maintain precise, confidential electronic case records in compliance with HIPAA and state regulations.
- Facilitate advocacy meetings and participate in community outreach initiatives to improve service accessibility.
- Mentor junior case managers and interns to foster a culture of professional growth and ethical practice.
Qualifications
- Master’s degree in Social Work (MSW) or a related field from an accredited institution.
- Active Licensed Clinical Social Worker (LCSW) or equivalent state licensure in Washington.
- Minimum of 3-5 years of direct social work or case management experience.
- Deep understanding of trauma-informed care and harm-reduction strategies.
- Strong interpersonal skills with the ability to build trust in diverse communities.
- Proficiency in case management software and Microsoft Office Suite.
- Ability to travel locally within the Seattle metropolitan area as required.
