Responsibilities
- Conduct comprehensive psychosocial and mental health assessments to evaluate patient and family needs.
- Provide clinical crisis intervention, emotional support, and short-term counseling to patients and families in distress.
- Design, coordinate, and execute complex discharge and transitional care plans in collaboration with the medical team.
- Advocate for patients' rights and facilitate access to vital public benefits, community resources, and support networks.
- Maintain precise, compliant, and timely documentation of all clinical interventions in the Electronic Health Record (EHR) system.
- Provide clinical mentorship and supervision to junior social workers and Master of Social Work (MSW) interns.
Qualifications
- Master's Degree in Social Work (MSW) from an institution accredited by the Council on Social Work Education (CSWE).
- Active Licensed Independent Clinical Social Worker (LICSW) license in the Commonwealth of Massachusetts.
- Minimum of 3 years of clinical social work experience, preferably within an acute care hospital or healthcare system.
- Demonstrated expertise in crisis intervention, grief counseling, and cognitive behavioral modalities.
- In-depth knowledge of community resource networks, Medicare/Medicaid, and long-term care regulations.
- Superb communication skills with the ability to collaborate effectively across multidisciplinary medical teams.
