Responsibilities
- Conduct thorough biopsychosocial assessments to identify client strengths and service needs.
- Develop, monitor, and adjust individualized service plans in collaboration with multi-disciplinary teams.
- Provide crisis intervention and short-term counseling for youth and family units.
- Maintain precise, confidential electronic health records (EHR) in compliance with state and federal regulations.
- Advocate for client access to essential services including housing, mental health, and educational support.
- Facilitate community resource referrals and coordinate care with external agencies.
- Participate in weekly case consultation meetings to review complex cases and outcomes.
Qualifications
- Master’s degree in Social Work (MSW) or a related Human Services field required.
- Minimum of 3 years of direct case management experience with youth and families.
- Current state licensure (e.g., LICSW, LMHC) is highly preferred.
- Strong understanding of trauma-informed care principles and intervention strategies.
- Excellent verbal and written communication skills with the ability to navigate diverse community settings.
- Proven ability to manage high caseloads while maintaining meticulous attention to detail.
- Proficiency with modern case management software and G-Suite tools.
