Responsibilities
- Develop and execute comprehensive case plans for families in crisis.
- Conduct regular home visits and community assessments to monitor progress.
- Facilitate connections between clients and essential government or private aid resources.
- Provide clinical crisis intervention and emotional support as needed.
- Maintain precise, confidential electronic records in compliance with state regulations.
- Collaborate with multidisciplinary teams including psychologists, educators, and legal advocates.
- Advocate for systemic improvements by participating in regional policy committees.
Qualifications
- Bachelor’s or Master’s Degree in Social Work, Psychology, or a related field.
- Minimum 3 years of professional experience in social services or case management.
- Current LSW or LCSW licensure in the state of Illinois is highly preferred.
- Strong interpersonal skills with the ability to maintain professional boundaries.
- Proficiency in case management software and Microsoft Office Suite.
- Proven ability to work effectively in high-stress, fast-paced environments.
- Valid driver’s license and reliable personal transportation.
