Responsibilities
- Develop and implement individualized service plans for at-risk youth and families.
- Conduct thorough psychosocial assessments and home visits to evaluate client needs.
- Provide crisis intervention and conflict resolution services in high-stress situations.
- Coordinate with schools, healthcare providers, and legal entities to ensure holistic care.
- Maintain precise, confidential electronic health records in compliance with state regulations.
- Facilitate group support sessions and educational workshops for community members.
- Advocate for policy changes and resources at the local governmental level.
Qualifications
- Master’s degree in Social Work (MSW) or a related field (Psychology, Counseling).
- LCSW or LSW licensure in the State of Illinois preferred.
- Minimum of 3 years of direct case management experience in a social services setting.
- Demonstrated proficiency in trauma-informed care methodologies.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Clean driving record and reliable transportation for community-based visits.
- Fluency in English and Spanish is highly desirable.
