Responsibilities
- Develop and implement individualized service plans for youth and family units.
- Conduct thorough intake assessments and regular follow-up monitoring for ongoing cases.
- Facilitate coordination between schools, healthcare providers, and legal entities.
- Maintain precise, high-quality documentation in our electronic case management system.
- Provide crisis intervention and de-escalation support as necessary.
- Conduct home visits to ensure safety standards and foster supportive environments.
- Lead periodic multidisciplinary team meetings to review case progress.
Qualifications
- Master's degree in Social Work (MSW), Counseling, or a related behavioral science field.
- Current LSW or LCSW licensure in the State of Illinois strongly preferred.
- Minimum of 3 years of direct case management experience in a social services setting.
- Demonstrated expertise in trauma-informed care and de-escalation techniques.
- Strong interpersonal skills with the ability to maintain professional boundaries.
- Valid driver’s license and reliable transportation for field visits.
- Proficiency with modern case management software and G-Suite tools.
