Responsibilities
- Develop and implement individualized service plans for high-risk families.
- Conduct thorough assessments to identify social, emotional, and economic needs.
- Coordinate with school districts, healthcare providers, and legal entities to ensure comprehensive support.
- Maintain precise clinical documentation in accordance with state and federal regulations.
- Facilitate crisis intervention sessions to de-escalate urgent family situations.
- Mentor junior case managers and contribute to team-wide professional development workshops.
- Represent the agency in community board meetings and networking initiatives.
Qualifications
- Master’s degree in Social Work (MSW), Psychology, or a related behavioral science field.
- Current LCSW or LSW licensure in the State of Illinois is required.
- Minimum of 3+ years of professional experience in direct social services or case management.
- Deep understanding of trauma-informed care and restorative justice practices.
- Proven ability to manage high-caseload environments with sensitivity and efficiency.
- Exceptional verbal and written communication skills.
- Valid driver's license and reliable transportation for community visits.
