Responsibilities
- Develop and execute individualized care plans for high-need youth and family units.
- Conduct thorough psychosocial assessments and home visits to monitor client progress and safety.
- Coordinate with local schools, medical providers, and legal entities to ensure comprehensive service delivery.
- Maintain precise, confidential case records and documentation in accordance with state compliance standards.
- Provide empathetic crisis counseling and de-escalation support during high-stress situations.
- Advocate for client needs during multi-disciplinary team meetings and court hearings.
- Mentor junior staff and participate in program development initiatives to improve service efficiency.
Qualifications
- Master’s degree in Social Work (MSW), Psychology, or a related human services field.
- Current LSW or LCSW licensure in the state of Illinois.
- Minimum of 3 years of experience in child welfare, foster care, or family preservation.
- Demonstrated proficiency in trauma-informed care and restorative justice practices.
- Excellent communication skills with the ability to build trust across diverse cultural backgrounds.
- Advanced proficiency in electronic health record (EHR) systems and data management.
- Valid driver’s license and reliable transportation for field visits.
