In this high-impact role, you will be the cornerstone of support for families in crisis, leveraging your clinical expertise to provide holistic case management and drive long-term stability.
Responsibilities
- Manage a dedicated caseload of at-risk youth, conducting thorough biopsychosocial assessments.
- Develop and implement individualized service plans (ISPs) focused on trauma-informed care.
- Collaborate with schools, medical professionals, and local agencies to advocate for client resources.
- Facilitate weekly family counseling sessions and conflict-resolution workshops.
- Maintain precise, confidential electronic case records in compliance with state regulations.
- Provide crisis intervention and on-call support as part of a rotating team schedule.
- Lead community outreach initiatives to improve access to essential social services.
Qualifications
- Master’s Degree in Social Work (MSW), Psychology, or a related field required.
- Active LCSW or LSW licensure in the state of Illinois.
- Minimum of 3 years of experience in child welfare or community-based social services.
- Demonstrated proficiency in crisis intervention and de-escalation techniques.
- Deep understanding of trauma-informed care models and child developmental psychology.
- Strong organizational skills with the ability to manage complex documentation requirements.
- Valid driver’s license and reliable personal transportation for field visits.
