Are you a dedicated advocate driven by the desire to create systemic change? Beacon Family Outreach Center is seeking a compassionate and strategic Senior Caseworker to join our Chicago-based team. In this role, you will provide intensive support to at-risk populations, coordinating essential services to foster independence and stability.
You will play a pivotal role in our multidisciplinary team, ensuring our clients receive high-quality, trauma-informed care in a supportive and professional environment.
Responsibilities
- Develop and implement comprehensive, individualized case plans for high-needs clients.
- Conduct thorough psychosocial assessments and crisis intervention as required.
- Facilitate inter-agency collaboration to ensure seamless access to housing, healthcare, and employment resources.
- Maintain precise, confidential electronic case records in compliance with state regulations.
- Lead periodic family team meetings to monitor progress and adjust care strategies.
- Provide mentorship and clinical guidance to junior case management staff.
- Advocate for client needs during community resource hearings and inter-departmental reviews.
Qualifications
- Master’s degree in Social Work (MSW), Psychology, or a related human services field.
- Current LCSW or LSW licensure in the state of Illinois.
- Minimum of 3-5 years of experience in clinical or community-based social work.
- Demonstrated expertise in trauma-informed care frameworks.
- Strong interpersonal skills with the ability to build trust in diverse communities.
- Proficiency in case management software and Microsoft Office Suite.
- Valid driver’s license and reliable transportation for community outreach.
