Responsibilities
- Develop and implement comprehensive, evidence-based care plans for high-risk individuals and families.
- Conduct thorough psychosocial assessments and identify underlying barriers to client stability.
- Maintain precise and ethical clinical documentation in accordance with state and federal regulations.
- Coordinate with multi-disciplinary teams, including healthcare providers, legal advocates, and housing authorities.
- Facilitate crisis intervention services during emergency situations to ensure immediate safety.
- Conduct regular home visits to monitor progress and provide essential on-site support.
- Advocate for client needs within public systems to ensure equitable access to community resources.
Qualifications
- Master’s Degree in Social Work (MSW), Psychology, or a related behavioral science field.
- Current LCSW or LSW licensure in the state of Illinois is highly preferred.
- Minimum of 3 years of experience in case management or community-based social services.
- Demonstrated expertise in trauma-informed care and crisis de-escalation techniques.
- Strong proficiency in utilizing electronic health record (EHR) systems for documentation.
- Valid driver's license and reliable transportation for field-based duties.
- Exceptional cultural competency and ability to work with diverse urban populations.
