Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized service plans tailored to the unique needs of clients.
- Provide direct crisis intervention, counseling, and emotional support to youth and their families.
- Advocate for clients within the healthcare, educational, and legal systems to ensure access to vital resources.
- Collaborate with interdisciplinary teams, including therapists, probation officers, and school administrators.
- Monitor client progress and adjust case plans as necessary to ensure successful outcomes.
- Document all case notes, interactions, and service delivery in the electronic health record system accurately and timely.
- Facilitate group workshops focused on life skills, anger management, and healthy relationships.
Qualifications
- Master’s degree in Social Work (MSW) from an accredited institution.
- Valid LCSW, LMSW, or LSW license in the State of New York.
- A minimum of 3-5 years of experience in social services, case management, or mental health counseling.
- Deep understanding of trauma-informed care principles and adolescent development.
- Excellent written and verbal communication skills, with the ability to write clear reports.
- Proficiency in Microsoft Office Suite and case management software (e.g., Salesforce, Credant).
- Bilingual skills (Spanish/English) are highly preferred.
