Responsibilities
- Conduct comprehensive biopsychosocial assessments and develop individualized case plans.
- Provide direct case management services, including crisis intervention and resource linkage.
- Facilitate therapeutic workshops and support groups focused on life skills and emotional resilience.
- Collaborate with schools, healthcare providers, and legal agencies to ensure holistic client care.
- Maintain accurate, up-to-date documentation in accordance with regulatory standards.
- Advocate for system-level changes to improve service delivery for the community.
Qualifications
- Master’s degree in Social Work (MSW) or a related field; LCSW preferred.
- Minimum of 3-5 years of progressive experience in social services or youth advocacy.
- Strong knowledge of local mental health and child welfare systems.
- Excellent interpersonal and communication skills.
- Ability to work effectively under pressure and handle sensitive information with discretion.
- Proficiency in Microsoft Office Suite and case management software (e.g., Salesforce, HMIS).
