Responsibilities
- Conduct comprehensive biopsychosocial assessments and develop individualized care plans for clients.
- Facilitate access to essential community resources, including housing, healthcare, and employment services.
- Monitor client progress and adjust care plans in accordance with changing needs and clinical goals.
- Maintain accurate and timely documentation in accordance with HIPAA and agency standards.
- Provide crisis intervention and emotional support to clients and their families during challenging times.
- Collaborate effectively with multidisciplinary teams, including medical professionals and legal advocates.
Qualifications
- Bachelor’s or Master’s degree in Social Work (MSW) required; LSW or LCSW license preferred.
- Minimum of 2-3 years of experience in case management or social work.
- Strong knowledge of community resources and social service systems.
- Excellent verbal and written communication skills.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Certified in CPR/First Aid (or willingness to obtain) is a plus.
