Responsibilities

  • Client Assessment & Planning: Conduct comprehensive intakes and psychosocial assessments to identify client needs and develop individualized care plans in alignment with evidence-based practices.
  • Service Coordination: Act as a primary liaison between clients and external agencies, coordinating housing, medical, mental health, and vocational services to ensure holistic support.
  • Advocacy: Advocate fiercely for client rights and access to resources, navigating complex bureaucracy to remove barriers to success.
  • Case Management: Manage a caseload of 20-30 active clients, providing direct counseling, crisis intervention, and ongoing case monitoring.
  • Documentation: Maintain accurate, timely, and confidential electronic health records (EHR) in compliance with HIPAA and agency standards.
  • Collaboration: Facilitate case conferences and multidisciplinary team meetings to share insights and refine treatment strategies.

Qualifications

  • Education: Master’s degree in Social Work (MSW) from an accredited university.
  • Licensure: Current Texas State Licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
  • Experience: Minimum of 3-5 years of experience in case management, social work, or a related human services field.
  • Knowledge: In-depth understanding of local community resources, housing programs, and healthcare systems in Travis County.
