Responsibilities
- Client Assessment & Planning: Conduct comprehensive intakes and psychosocial assessments to identify client needs and develop individualized care plans in alignment with evidence-based practices.
- Service Coordination: Act as a primary liaison between clients and external agencies, coordinating housing, medical, mental health, and vocational services to ensure holistic support.
- Advocacy: Advocate fiercely for client rights and access to resources, navigating complex bureaucracy to remove barriers to success.
- Case Management: Manage a caseload of 20-30 active clients, providing direct counseling, crisis intervention, and ongoing case monitoring.
- Documentation: Maintain accurate, timely, and confidential electronic health records (EHR) in compliance with HIPAA and agency standards.
- Collaboration: Facilitate case conferences and multidisciplinary team meetings to share insights and refine treatment strategies.
- Program Development: Contribute to the continuous improvement of our social service programs by providing feedback based on client outcomes.
Qualifications
- Education: Master’s degree in Social Work (MSW) from an accredited university.
- Licensure: Current Texas State Licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
- Experience: Minimum of 3-5 years of experience in case management, social work, or a related human services field.
- Knowledge: In-depth understanding of local community resources, housing programs, and healthcare systems in Travis County.
- Skills: Strong interpersonal communication skills with the ability to build rapport quickly and effectively.
- Computer Proficiency: Advanced proficiency in Microsoft Office Suite and Electronic Medical Records (EMR) software.
- Personal Attributes: High level of cultural competency, resilience, and the ability to handle high-stress situations with grace.
