Responsibilities
- Develop and implement personalized case management plans for at-risk populations
- Coordinate multidisciplinary support services including housing, healthcare, and employment resources
- Conduct comprehensive psychosocial assessments and crisis interventions
- Advocate for clients within community systems and legal frameworks
- Maintain detailed documentation and compliance with state/federal regulations
- Lead community outreach programs to address systemic barriers
Qualifications
- Master's degree in Social Work, Psychology, or related field required
- 5+ years progressive case management experience in social services
- Licensure (LCSW or equivalent) in the state of Illinois
- Proven crisis intervention and trauma-informed care expertise
- Advanced knowledge of Chicago's social services ecosystem
- Exceptional cultural competency and bilingual (Spanish) proficiency
- Data analysis experience for outcome measurement
