Responsibilities
- Develop and implement individualized service plans for at-risk populations including youth, families, and homeless individuals
- Conduct comprehensive needs assessments and risk evaluations using standardized protocols
- Coordinate multidisciplinary care teams including healthcare providers, legal advocates, and educators
- Provide crisis intervention and trauma-informed counseling in emergency situations
- Monitor program outcomes through data collection and quarterly impact reports
- Train and supervise junior case staff on best practices in social work
- Forge community partnerships with schools, shelters, and government agencies
Qualifications
- Master's degree in Social Work (MSW) or related human services field
- LCSW/LISW licensure in Illinois with 3+ years post-licensure experience
- Proven expertise in crisis intervention and trauma-informed methodologies
- Advanced proficiency in case management software (e.g., Cerner, CareLogic)
- Valid Illinois driver's license with reliable transportation
- Bilingual proficiency in Spanish preferred
- Certification in Mental Health First Aid required
- Experience with grant-funded program implementation and reporting
