Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care plans.
- Provide crisis intervention and emotional support to clients facing complex challenges.
- Connect clients with community resources, housing, employment, and healthcare services.
- Monitor client progress and adjust care plans as necessary to ensure positive outcomes.
- Document all interactions and case details in accordance with agency policies and legal standards.
- Collaborate with interdisciplinary teams, including healthcare providers and legal professionals.
Qualifications
- Bachelor’s degree in Social Work (BSW) or a related field (Masters preferred).
- Valid registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum of 3-5 years of experience in case management or social services.
- Strong knowledge of community resources and social policy.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a caseload effectively.
- Proficiency in Microsoft Office Suite and case management software.
