Key Responsibilities:

- Conduct comprehensive psychosocial assessments and develop individualized care plans tailored to client needs.
- Facilitate access to vital community resources, including emergency housing, medical care, and mental health services.
- Provide crisis intervention and emotional support to clients and their families during high-stress situations.
- Advocate for client rights and navigate complex social service systems on their behalf.
- Maintain accurate, confidential, and up-to-date case files in compliance with regulatory standards.
- Collaborate with external agencies and community partners to expand service offerings.
- Lead weekly case supervision meetings and mentor junior staff members.
Qualifications
- Master’s degree in Social Work (MSW) preferred; Bachelor’s degree with relevant experience accepted.
- Minimum of 3-5 years of professional experience in case management or social services.
- Valid New York State driver’s license and reliable transportation required.
- Strong understanding of New York’s social safety net and local community organizations.
- Excellent verbal and written communication skills with a focus on cultural competency.
- Ability to handle sensitive information with the highest level of confidentiality.
- Proficiency in Microsoft Office Suite and case management software.
