Responsibilities
- Develop and implement individualized service plans for clients facing housing instability, mental health challenges, and economic hardship
- Coordinate with community partners to connect clients with healthcare, employment, and educational resources
- Provide crisis intervention and ongoing support through home visits and virtual check-ins
- Maintain accurate case documentation while ensuring HIPAA compliance and data security
- Lead outreach initiatives to identify at-risk populations and expand service accessibility
- Collaborate with social workers, therapists, and legal advocates to address complex client needs
- Train junior staff on trauma-informed care methodologies and cultural competency
Qualifications
- Master's degree in Social Work, Psychology, or related field (MSW preferred)
- 5+ years of experience in direct client services with vulnerable populations
- Licensure in Illinois (LCSW or equivalent)
- Proven expertise in crisis intervention and case management software
- Strong understanding of Chicago's social services ecosystem and resource networks
- Certification in Trauma-Focused Cognitive Behavioral Therapy
- Ability to manage caseloads of 25+ clients while maintaining documentation standards
- Fluency in Spanish or other relevant languages (highly valued)
