Responsibilities
- Develop and execute comprehensive care plans for 25+ clients facing housing instability and mental health challenges
- Collaborate with cross-functional teams including therapists, housing specialists, and legal advocates
- Conduct crisis intervention and safety planning for high-risk situations
- Analyze community data to identify systemic barriers and inform program improvements
- Maintain meticulous documentation in compliance with state and federal regulations
- Mentor junior staff through structured supervision and peer learning sessions
- Coordinate with external partners including healthcare providers and government agencies
Qualifications
- Master's degree in Social Work (MSW) with LCSW/LCSW-P preferred
- 3+ years direct case management experience in urban social services
- Proven crisis intervention skills with vulnerable adult populations
- Advanced knowledge of Chicago's social service resources and systems
- Data analysis experience with client outcome tracking tools
- Cultural competency in serving diverse communities (LGBTQ+, immigrant, etc.)
- Valid Illinois driver's license and reliable transportation
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) a plus
