Responsibilities
- Conduct comprehensive psychosocial assessments to identify client needs and develop individualized care plans.
- Provide crisis intervention and emotional support to clients facing acute challenges.
- Connect clients with essential resources, including housing assistance, healthcare, and employment services.
- Monitor client progress and adjust care plans as necessary to ensure positive outcomes.
- Advocate for client rights and ensure access to appropriate community services.
- Maintain accurate, confidential case files and documentation in compliance with state regulations.
- Collaborate with multidisciplinary teams to provide holistic support.
Qualifications
- Master’s degree in Social Work (MSW) or a related field required.
- Valid New York State Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) credential.
- Minimum of 2 years of experience in case management or social work.
- Strong knowledge of local and federal social service programs and policies.
- Exceptional communication and interpersonal skills.
- Ability to work independently and manage a high caseload effectively.
- Bilingual (Spanish/English) preferred but not required.
