Responsibilities
- Conduct comprehensive psychosocial assessments and care plan reviews for new and existing clients.
- Develop and implement individualized support plans in line with Care Act requirements.
- Monitor client progress and adapt strategies to meet changing needs.
- Act as the primary point of contact for stakeholders, including family members and external agencies.
- Provide crisis intervention and emotional support during acute episodes.
- Maintain accurate, up-to-date case records and administrative documentation.
- Facilitate support groups and workshops to promote community integration.
Qualifications
- Minimum of a Bachelor’s degree in Social Work, Psychology, or a related field.
- Current registration with the Health and Care Professions Council (HCPC) or equivalent.
- Proven experience in social casework, case management, or a similar role.
- Strong understanding of safeguarding policies and procedures.
- Excellent verbal and written communication skills.
- Ability to work independently and manage a caseload effectively.
- Proficiency in Microsoft Office Suite and case management software.
