We are committed to bridging the gap between resources and those in need. As a Senior Case Manager, you will be the voice for our clients, navigating complex systems to secure housing, healthcare, and employment.
Responsibilities
- Conduct comprehensive client assessments to identify social, emotional, and financial needs.
- Develop, implement, and monitor individualized care plans in collaboration with clients and multidisciplinary teams.
- Advocate for clients to ensure access to essential services, including housing, healthcare, and legal aid.
- Maintain accurate and up-to-date case files and documentation in compliance with agency and state regulations.
- Crisis intervention and conflict resolution support for individuals facing emergencies.
- Facilitate support groups and workshops to foster community building and skill development.
- Collaborate with external agencies and stakeholders to expand service networks.
Qualifications
- Bachelor’s degree in Social Work (BSW), Psychology, or a related field (Master’s degree preferred).
- Current licensure as an LCSW, LMFT, LMHC, or LCSW candidacy.
- Minimum of 3-5 years of experience in case management, social work, or a related human services field.
- Strong knowledge of local social service systems, community resources, and government programs.
- Excellent verbal and written communication skills, with the ability to build rapport quickly.
- Demonstrated ability to handle sensitive information with strict confidentiality and ethics.
- Proficiency in Microsoft Office Suite and case management software.
