Responsibilities
- Develop and implement comprehensive care plans for 25+ vulnerable clients across housing, employment, and mental health domains
- Conduct biopsychosocial assessments and crisis interventions using trauma-informed methodologies
- Collaborate with multidisciplinary teams including healthcare providers, legal advocates, and government agencies
- Monitor and document client progress using EHR systems while maintaining HIPAA compliance
- Provide advocacy and resource navigation for clients facing systemic barriers
- Mentor junior staff through case consultation and best practice training
Qualifications
- Master's degree in Social Work, Psychology, or related field required
- LCSW/LPC licensure in Illinois with 3+ years clinical experience
- Proven expertise in crisis intervention and trauma-informed care models
- Advanced proficiency in electronic health records (e.g., Epic, Cerner)
- Certification in Trauma-Focused Cognitive Behavioral Therapy preferred
- Demonstrated cultural competency serving diverse urban populations
- Valid Illinois driver's license and reliable transportation
