Responsibilities
- Conduct comprehensive client assessments and develop individualized care plans.
- Advocate for clients to ensure access to healthcare, housing, and employment services.
- Monitor client progress and adjust interventions as necessary to meet changing needs.
- Collaborate with multidisciplinary teams, including medical staff and legal professionals.
- Maintain accurate and confidential case records in compliance with HIPAA regulations.
- Provide crisis intervention and emotional support to vulnerable populations.
Qualifications
- Masters in Social Work (MSW) required; LCSW or LMSW preferred.
- Minimum of 3-5 years of experience in case management or social services.
- Strong knowledge of community resources and social service systems.
- Excellent verbal and written communication skills.
- Ability to handle sensitive information with the highest level of confidentiality.
- Valid New York State driver’s license and reliable transportation.
