Responsibilities
- Develop and implement individualized case management plans for families experiencing homelessness, domestic violence, or economic hardship
- Provide crisis intervention, counseling, and advocacy services using evidence-based modalities (CBT, Motivational Interviewing)
- Coordinate community resources including housing vouchers, healthcare access, and employment support
- Maintain detailed case documentation in compliance with state/federal regulations and agency standards
- Conduct home visits and community outreach to build trust and assess client needs
- Collaborate with schools, healthcare providers, and legal partners to create integrated support networks
- Mentor junior staff and contribute to program evaluation and quality improvement initiatives
Qualifications
- Master's degree in Social Work (MSW) from an accredited program
- LCSW licensure in Illinois with 3+ years post-MSW clinical experience
- Proven expertise in family systems theory and trauma-informed care practices
- Strong crisis intervention skills with documented success in high-stakes situations
- Experience navigating public benefits systems (SNAP, TANF, Medicaid) and housing vouchers
- Bilingual proficiency in Spanish preferred
- Valid Illinois driver's license and reliable transportation for home visits
- Proficiency in electronic health records (EHR) systems and data management
