Responsibilities
- Develop and implement comprehensive case management plans for clients facing homelessness, mental health challenges, and economic hardship
- Collaborate with multidisciplinary teams including healthcare providers, legal advocates, and government agencies
- Conduct thorough psychosocial assessments and risk evaluations
- Provide crisis intervention and trauma-informed support services
- Maintain accurate documentation and reporting for compliance with state regulations
- Lead community outreach programs to connect underserved populations with resources
- Mentor junior staff and supervise practicum students
Qualifications
- Master's degree in Social Work, Psychology, or related field required
- Minimum 5 years of case management experience with vulnerable populations
- Licensure in Illinois (LCSW or LPC)
- Expertise in trauma-informed care and crisis intervention protocols
- Proficiency in electronic health records (EHR) systems
- Strong cultural competency and ability to work with diverse communities
- Valid Illinois driver's license and reliable transportation
- CPR/First Aid certification
