Responsibilities
- Conduct comprehensive biopsychosocial assessments and develop individualized service plans (ISP) for clients.
- Advocate for clients within healthcare, legal, and government systems to ensure access to necessary resources.
- Monitor client progress and adjust service plans as needs evolve.
- Facilitate support groups and psycho-education workshops.
- Maintain accurate, timely documentation in electronic health records (EHR).
- Collaborate with interdisciplinary teams to provide holistic care.
Qualifications
- Master’s degree in Social Work (MSW) or related field required; LCSW or LMSW preferred.
- Minimum of 3-5 years of experience in case management, social work, or community health.
- Valid New York State Social Work license or eligibility to obtain licensure.
- Strong knowledge of local and federal social service policies and entitlement programs.
- Excellent verbal and written communication skills.
- Proficiency in Microsoft Office Suite and case management software.
