Responsibilities
- Develop and implement comprehensive case management plans for clients facing homelessness, mental health challenges, and economic instability
- Conduct psychosocial assessments and crisis interventions while maintaining rigorous documentation standards
- Coordinate interdisciplinary services including housing navigation, vocational training, and healthcare access
- Build partnerships with community organizations, government agencies, and healthcare providers to expand resource networks
- Provide trauma-informed counseling and advocacy for vulnerable populations including veterans, LGBTQ+ youth, and immigrants
- Lead outreach initiatives in high-need neighborhoods to identify and engage at-risk individuals
- Maintain compliance with NYS OMH regulations and organizational quality assurance protocols
Qualifications
- Master's degree in Social Work (MSW) with NY LMSW license required
- Minimum 3 years of direct case management experience in urban social services
- Certification in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) preferred
- Proven ability to navigate complex systems including Medicaid, SNAP, and housing vouchers
- Experience developing culturally responsive interventions for diverse populations
- Advanced proficiency in electronic health records (EHR) systems and data analysis tools
- Exceptional crisis management skills with demonstrated de-escalation techniques
- Valid NYS driver's license with clean driving record
