Responsibilities
- Develop and implement comprehensive care plans for clients facing homelessness, mental health challenges, and substance abuse disorders
- Coordinate multidisciplinary services including housing assistance, vocational training, and crisis intervention
- Conduct biopsychosocial assessments to identify client needs and barriers to self-sufficiency
- Advocate for clients within healthcare, legal, and social service systems
- Maintain detailed case documentation while maintaining HIPAA compliance
- Lead trauma-informed support groups and community outreach initiatives
- Analyze outcomes data to continuously improve service delivery models
Qualifications
- Master's degree in Social Work, Psychology, Counseling, or related field
- LCSW/LPC licensure or eligibility for Illinois licensure
- 3+ years direct case management experience with vulnerable populations
- Proficiency in electronic health records (EHR) systems
- Certified in Trauma-Focused Cognitive Behavioral Therapy (TF-CBT)
- Strong crisis intervention and de-escalation skills
- Demonstrated ability to build rapport across diverse cultural backgrounds
- Valid Illinois driver's license with reliable transportation
