Responsibilities
- Develop and implement individualized care plans for clients facing complex social barriers
- Coordinate interdisciplinary services including housing, healthcare, and employment support
- Conduct comprehensive psychosocial assessments and crisis interventions
- Build partnerships with community organizations, government agencies, and healthcare providers
- Maintain detailed case documentation compliant with funder requirements
- Provide trauma-informed counseling and advocacy for clients navigating systemic challenges
- Lead team training on cultural competency and evidence-based practices
Qualifications
- Master's degree in Social Work, Psychology, or related field (MSW preferred)
- 5+ years progressive experience in direct social services case management
- Licensure in Illinois (LCSW or LPC) or eligibility within 6 months
- Proven success in housing-first and harm reduction methodologies
- Advanced knowledge of Chicago's social services ecosystem and resources
- Exceptional crisis intervention and de-escalation skills
- Experience supervising case managers or interns
- Bilingual English/Spanish proficiency required
