Responsibilities
- Develop and implement comprehensive case management plans for clients facing housing instability, mental health challenges, and economic hardship
- Conduct thorough psychosocial assessments and coordinate multi-agency interventions
- Provide crisis intervention and ongoing emotional support to high-risk populations
- Collaborate with healthcare providers, legal advocates, and government agencies to create holistic support networks
- Maintain accurate client documentation while ensuring strict confidentiality and compliance with HIPAA regulations
- Mentor junior staff and lead community outreach initiatives
- Analyze service gaps and propose program improvements based on client outcomes
Qualifications
- Master's degree in Social Work, Psychology, or related field (MSW preferred)
- Minimum 5 years of experience in direct client services with vulnerable populations
- Licensure in New York State (LMSW or LCSW required)
- Proven expertise in trauma-informed care and crisis intervention techniques
- Strong cultural competency in serving diverse urban communities
- Advanced proficiency in case management software (e.g., CareLogic, Salesforce)
- Excellent written and verbal communication skills with ability to advocate for clients
- Valid NYS driver's license and reliable transportation for home visits
