Responsibilities
- Develop and implement individualized service plans for clients facing housing instability, mental health challenges, and economic hardship
- Coordinate multi-agency interventions including healthcare providers, legal advocates, and employment specialists
- Conduct comprehensive psychosocial assessments using trauma-informed methodologies
- Provide crisis intervention and safety planning for high-risk populations
- Lead community outreach initiatives to identify and engage marginalized groups
- Maintain meticulous documentation compliant with state regulations and funding requirements
- Mentor junior staff and supervise practicum students in case management best practices
Qualifications
- Master's degree in Social Work, Counseling, or related field (LCSW/LPC preferred)
- Minimum 5 years progressive experience in direct social services case management
- Demonstrated expertise in crisis intervention and trauma-informed care
- Proficiency in electronic health records (EHR) and case management software
- Valid Illinois driver's license with reliable transportation for home visits
- Bilingual proficiency in Spanish strongly preferred
- Proven ability to navigate complex community systems and build collaborative partnerships
- Certification in Trauma-Focused Cognitive Behavioral Therapy a plus
