Responsibilities
- Develop and implement individualized service plans for clients facing housing instability, trauma, and mental health challenges
- Conduct comprehensive needs assessments and coordinate care with healthcare providers, legal advocates, and community partners
- Facilitate crisis intervention and de-escalation for high-risk situations using trauma-informed techniques
- Document client progress meticulously in electronic health records while maintaining strict confidentiality standards
- Lead outreach initiatives to identify underserved populations and connect them with critical resources
- Collaborate with housing authorities to secure stable accommodations for chronically homeless individuals
- Advocate for systemic policy changes through participation in community coalitions and task forces
Qualifications
- Master's degree in Social Work, Psychology, Counseling, or related field
- 3+ years of direct case management experience with vulnerable populations
- Licensure (LCSW, LCPC, or equivalent) in Illinois
- Certification in Trauma-Focused Cognitive Behavioral Therapy preferred
- Advanced proficiency in electronic health record systems (e.g., Cerner, Epic)
- Proven success in navigating complex social service systems and securing housing resources
- Experience supervising interns or junior staff
- Bilingual proficiency in Spanish or Polish strongly preferred
