Responsibilities
- Develop and implement individualized case management plans for 25+ high-need clients
- Coordinate multidisciplinary services including housing, healthcare, and employment support
- Conduct comprehensive assessments and crisis interventions 24/7 on-call rotation
- Advocate for clients within legal systems, healthcare facilities, and government agencies
- Maintain detailed case documentation compliant with federal and state regulations
- Facilitate therapeutic group sessions and community workshops
- Collaborate with community partners to expand resource access and service delivery
Qualifications
- Master's degree in Social Work, Psychology, or related field required
- LCSW or LMSW license in New York State mandatory
- Minimum 5 years case management experience with vulnerable populations
- Expertise in trauma-informed care and crisis intervention protocols
- Advanced knowledge of NYC social services systems and resources
- Proficiency with electronic health records (EHR) systems
- Valid driver's license and reliable transportation for field visits
- Bilingual Spanish proficiency highly preferred
