Responsibilities
- Develop and implement comprehensive case management plans for 15-20 high-risk clients
- Conduct psychosocial assessments and coordinate multidisciplinary support services
- Advocate for clients within healthcare, legal, and social service systems
- Provide crisis intervention and trauma-informed counseling sessions
- Collaborate with community partners to expand resource accessibility
- Maintain detailed case documentation and outcome metrics
- Lead outreach initiatives in underserved Toronto neighborhoods
Qualifications
- Master's degree in Social Work, Psychology, or related field
- Minimum 3 years direct case management experience
- Certified Clinical Counselor (CCC) or Ontario College of Social Workers (OCSWSSW) registration
- Expertise in harm reduction and trauma-informed care models
- Proven success in navigating complex community resources
- Strong crisis intervention and de-escalation skills
- Familiarity with Toronto's social services landscape
- Valid Ontario driver's license and reliable vehicle
