Responsibilities
- Develop and implement individualized service plans for clients facing homelessness, mental health challenges, and economic barriers
- Coordinate multidisciplinary teams including healthcare providers, legal advocates, and housing specialists
- Conduct comprehensive needs assessments and crisis interventions in high-pressure situations
- Maintain accurate documentation while maintaining HIPAA compliance and ethical standards
- Facilitate life-skills workshops and connect clients to community resources
- Advocate for systemic policy changes affecting underserved populations
Qualifications
- Master's degree in Social Work, Psychology, or related field
- 3+ years direct experience in case management with vulnerable populations
- Licensure (LICSW, LMHC, or equivalent) in Washington State
- Proven crisis intervention and de-escalation techniques
- Advanced knowledge of local social service systems and resources
- Strong cultural competency serving diverse populations
- Data-driven mindset with experience in outcome measurement
