Responsibilities
- Develop and implement individualized service plans for at-risk populations including youth, veterans, and low-income families
- Conduct comprehensive needs assessments and connect clients with community resources (housing, healthcare, employment)
- Provide crisis intervention and trauma-informed counseling during emergency situations
- Collaborate with multidisciplinary teams to coordinate care and ensure service continuity
- Maintain detailed case documentation compliant with NYS regulations and agency standards
- Lead community outreach initiatives to expand program accessibility and impact
- Mentor junior staff and contribute to agency-wide policy improvements
Qualifications
- Master's degree in Social Work, Psychology, or related field (LMSW/LCSW preferred)
- 3+ years direct case management experience with vulnerable populations
- Crisis intervention certification and trauma-informed care training
- Proficiency in electronic health records (EHR) systems and documentation protocols
- Exceptional communication skills with diverse cultural and socioeconomic backgrounds
- Valid NYS driver's license and reliable transportation for home visits
- Experience with grant reporting and outcome measurement
