Responsibilities
- Develop and implement comprehensive case management plans for vulnerable individuals and families
- Conduct thorough psychosocial assessments and coordinate multidisciplinary support services
- Provide crisis intervention and trauma-informed counseling to diverse client populations
- Collaborate with community partners, healthcare providers, and government agencies
- Maintain detailed client documentation while maintaining HIPAA compliance
- Lead outreach initiatives to identify and engage at-risk community members
- Monitor client progress and adjust intervention strategies as needed
Qualifications
- Master's degree in Social Work, Psychology, or related field (MSW preferred)
- Minimum 3 years progressive case management experience in social services
- Licensure in Illinois (LCSW, LPC, or LCPC)
- Proven crisis intervention and trauma-informed care expertise
- Experience with electronic health records and documentation systems
- Cultural competency working with diverse populations
- Valid Illinois driver's license and reliable transportation
- Strong analytical skills and ability to manage complex caseloads
