Responsibilities
- Develop and implement comprehensive case management plans for 25+ high-needs clients
- Coordinate multidisciplinary services including housing, healthcare, and employment support
- Conduct thorough psychosocial assessments and crisis intervention
- Mentor 3-5 junior case managers through supervision and training
- Collaborate with community partners to expand resource networks
- Maintain accurate documentation and outcome reporting for funding compliance
- Lead outreach initiatives to identify and engage at-risk populations
Qualifications
- Master's degree in Social Work (MSW) or related field
- 3+ years direct case management experience in social services
- Licensure (LCSW or equivalent) in Illinois
- Expertise in trauma-informed care and cultural competency
- Strong assessment, documentation, and crisis management skills
- Experience supervising staff or interns
- Valid driver's license and reliable transportation
