Responsibilities
- Develop and implement personalized case management plans for at-risk individuals and families
- Conduct comprehensive psychosocial assessments and coordinate multi-agency support services
- Provide crisis intervention and trauma-informed counseling for vulnerable populations
- Collaborate with healthcare providers, schools, and community organizations to create support networks
- Maintain detailed case documentation compliant with state and federal regulations
- Lead community workshops on mental health awareness and resource navigation
- Supervise junior case managers and interns through structured mentorship programs
Qualifications
- Master's degree in Social Work, Psychology, or related field (MSW preferred)
- Minimum 3 years of direct case management experience in urban settings
- Licensure in Illinois (LCSW or LSW required)
- Expertise in trauma-informed care and crisis intervention protocols
- Proficiency with electronic health records (EHR) systems like Epic or Cerner
- Strong cultural competency serving diverse Chicago communities
- Valid Illinois driver's license and reliable transportation
- Experience supervising social work staff or interns
