Responsibilities
- Develop and implement individualized care plans for 15-20 clients facing housing instability, mental health crises, and systemic barriers
- Conduct comprehensive needs assessments and coordinate multi-agency interventions (housing, legal, healthcare)
- Provide crisis intervention and de-escalation support during emergency situations
- Maintain detailed client documentation and progress reports using our proprietary case management platform
- Facilitate support groups and community workshops on financial literacy and parenting skills
- Collaborate with policy team to advocate for systemic changes affecting vulnerable populations
- Mentor junior case managers and contribute to agency-wide quality improvement initiatives
Qualifications
- Master's degree in Social Work, Psychology, or related field required (LCSW/LMSW preferred)
- Minimum 5 years direct case management experience in urban social services setting
- Expertise in trauma-informed care and crisis intervention protocols
- Proficiency with electronic health records (EHR) systems and data-driven reporting
- Valid New York State driver's license and reliable transportation
- Bilingual fluency in Spanish or Mandarin strongly preferred
- Certification in Mental Health First Aid and Harm Reduction principles
- Proven track record of successful housing stabilization and resource navigation
