Responsibilities
- Assess client needs and develop comprehensive case management plans
- Connect clients with community resources, housing assistance, and mental health services
- Monitor client progress and adjust intervention strategies as needed
- Maintain detailed case documentation and ensure compliance with agency policies
- Collaborate with interdisciplinary teams including healthcare providers and legal advocates
- Provide crisis intervention and emergency support services
- Conduct home visits and community outreach to build trust and accessibility
Qualifications
- Master's degree in Social Work, Psychology, or related field
- 3+ years of direct case management experience in social services
- Licensure in Illinois (LCSW, LPC, or equivalent)
- Strong crisis intervention and conflict resolution skills
- Proficiency with electronic health records and case management software
- Cultural competency and experience working with diverse populations
- Valid Illinois driver's license and reliable transportation
