Responsibilities
- Develop and implement individualized care plans for clients facing housing instability, mental health challenges, and economic hardship
- Collaborate with multidisciplinary teams including healthcare providers, legal advocates, and government agencies
- Conduct comprehensive psychosocial assessments and crisis interventions for high-risk populations
- Manage caseloads of 25+ clients while maintaining documentation compliance with state regulations
- Lead community workshops on financial literacy, housing rights, and wellness strategies
- Analyze systemic barriers and propose policy recommendations to organizational leadership
Qualifications
- Master's degree in Social Work, Psychology, or related field (MSW preferred)
- 3+ years of direct case management experience with vulnerable adult populations
- Licensure in Washington State (LSW, LMHC, or equivalent)
- Expertise in trauma-informed care and crisis de-escalation techniques
- Proficiency with electronic health records (EHR) systems and data-driven reporting
- Bilingual proficiency in Spanish or Somali strongly preferred
- Valid Washington State driver's license and reliable transportation
