Responsibilities
- Develop and implement comprehensive care plans for clients facing homelessness, mental health challenges, and economic instability
- Coordinate multidisciplinary services including housing vouchers, healthcare access, and employment support
- Conduct biopsychosocial assessments and crisis interventions for high-risk individuals
- Maintain detailed case documentation compliant with state and federal regulations
- Liaise with government agencies, healthcare providers, and community organizations
- Provide trauma-informed counseling and advocacy for underserved populations
- Analyze client outcomes and refine service delivery strategies
Qualifications
- Master's degree in Social Work, Psychology, or related field required
- 3+ years of direct case management experience in urban social services
- Licensure in Washington State (LICSW, LMHC, or equivalent)
- Proven crisis intervention and de-escalation skills
- Expertise in navigating HUD regulations and Medicaid systems
- Cultural humility and experience serving diverse populations
- Advanced data literacy for outcome tracking and reporting
- Valid Washington driver's license and reliable transportation
